Patient Instructions for Using HSA/FSA to Pay for Clara Recovery

Instructions

Clara Recovery is a dietary supplement formulated to support nutritional needs before and after surgery. While supplements are not automatically covered by Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA), you may be able to use HSA/FSA funds if your healthcare provider determines it is medically necessary for your recovery.

Follow the steps below to request reimbursement:

Step 1:

Print the Letter of Medical Necessity Form

Step 2:

Ask Your Doctor to Complete the Form

  • Take the form to your surgeon, physician, or other provider who is involved in your surgical care.

  • Ask them to:

    • Complete and sign the form.

    • Provide their NPI number and contact information.

Step 3:

Save Your Purchase Receipt

  • Keep the email or printed receipt from your Clara Recovery order.
    You will need this for reimbursement.

Step 4:

Submit to Your HSA/FSA Administrator

  • Send the following to your HSA/FSA plan provider:

    • A copy of the completed LMN form

    • Your purchase receipt

    • Any required reimbursement form from your plan

  • Contact your HSA/FSA administrator or benefits department if you're unsure how to submit.
